Switch medication from clomipramine to nortriptyline.[1] [2]
Stop clomipramine - Before day 0: gradually reduce dosage of clomipramine to a maximum of 75 mg/day, when this dosage is > 75 mg/day.
- Day 1: reduce dosage of clomipramine to 50 mg/day.
- Day 8: reduce dosage of clomipramine to 25 mg/day.
- Day 15: stop administration of clomipramine.
Start nortriptylineDay 8: start administration of nortriptyline in a dosage of 25 mg/day.
- Day 15: increase dosage of nortriptyline to 50 mg/day.
- Day 22: increase dosage of nortriptyline.
