Switch medication from clomipramine to agomelatine.[1] [2]
- Before day 1: gradually reduce dosage of clomipramine to a maximum of 75 mg/day, when this dosage is > 75 mg/day.
- Day 1: reduce dosage of clomipramine to 50 mg/day.
- Day 8: reduce dosage of clomipramine to 25 mg/day.
- Day 15: stop administration of clomipramine.
- Day 1: simultaneously start administration of agomelatine in a dosage of 25 mg/day.
- Day 15: If necessary, increase dosage of agomelatine to 50 mg/day.
