Switch medication from mirtazapine to vortioxetine.[1] [2]
- Before day 1: gradually reduce dosage of mirtazapine to a maximum of 30 mg/day, when this dosage is > 30 mg/day.
- Day 1: reduce the dosage of 30 mg/day to 15 mg/day.
- Day 8: stop administration of mirtazapine.
- A wash-out period is not necessary.
- Day 8: start administration of vortioxetine in a dosage of 10 mg/day.
- Occurrence of serotonin syndrome is theoretically possible, so caution is necessary.
