Switch medication from fluoxetine to mirtazapine.[1] [2]
- Reduce the dosage to 20mg/day and then stop the administration
No wash-out period is needed.
- Day 2: start mirtazapine the next day in low dosage of 15 mg/day.
- Day 22: increase dosage of mirtazapine to normal, 30 mg/day.
- (Desmethyl)Fluoxetine has a very long elimination time; phasing out fluoxetine is therefore not necessary.
