Switch medication from mirtazapine to venlafaxine.[1] [2]
- Day 1: Decrease dose to 50%
- Day 8: Stop
- Day 1: Start with 50% of the target dose
- Day 8: Increase dose to 100% of the target dose
- Day 9 and after: Gradually increase dose as necessary
- Safe target dose venlafaxine = 75 mg
- Occurrence of the serotonin syndrome is not likely, but theoretically possible, so caution is necessary.
