Switch medication from trazodone to tranylcypromine.[1] [2]
Stop trazodone - Before day 1: gradually reduce dosage of trazodone to a maximum of 150 mg/day, when this dosage is > 150 mg/day.
- Day 1: reduce dosage of trazodone to 100 mg/day.
- Day 4: stop administration of trazodone.
Start tranylcypromine- Only a washout of 3 days (so start at Day 4) is necessarily when the dose of trazodone is >100 mg. When the dose is =< 100mg tranylcypromine can be started immediately
- Start tranylcypromine in a low dosage of 20 mg/day for one week.
- After this week the dosage of tranylcypromine can be increased.
More information- Occurrence of the serotonin syndrome is not likely, but theoretically possible, so caution is necessary.
