Switch medication from dosulepine to sertraline.[1] [2]
- Before day 1: gradually reduce dosage of dosulepine to a maximum of 75 mg/day.
- Day 1: reduce dosage of dosulepine to 50 mg/day.
- Day 3: reduce dosage of dosulepine to 25 mg/day.
- Day 7: stop administration of dosulepine.
- Day 8: start administration of sertraline in a normal dosage of 50 mg/day.
- Day 22: if necessary increase dosage of sertraline to 100 mg/day.
- Occurrence of serotonin syndrome is theoretically possible, so caution is necessary.
- Sertraline is a weak inhibitor of CYP2D6, which metabolizes dosulepine.
