Switch medication from nortriptyline to trazodone.[1] [2]

  • Before day 0: gradually reduce dosage of nortriptyline to a maximum of 50 mg/ day, when this dosage is > 50 mg/day.
  • Day 1: reduce dosage of nortriptyline to 25 mg/day.
  • Day 8: stop administration of nortriptyline.

No wash-out period is needed.

  • Day 1: simultaneously start administration of trazodone in a dosage of 150 mg/day.
  • Day 8: continue only administration of trazodone. If necessary, increase the dosage of trazodone to a maximum of 400 mg/day.
