Switch medication from moclobemide to nortriptyline.[1] [2]

  • Before day 0: gradually reduce dosage of moclobemide to a maximum of 300 mg/day.
  • Day 1: reduce the dosage of moclobemide to 150 mg/day.
  • Day 8: stop administration of moclobemide.

  • No wash-out period is necessary.
  • Day 9: start nortriptyline after this period in a normal dosage of 50 mg/day.
  • “Start low, go slow” is not required, but caution is necessary.
