Stop amitriptyline
- Before day 0: gradually reduce dosage of amitriptyline to a maximum of 100 mg/ day, when this dosage is > 100 mg/day.
- Day 1-3: reduce dosage of amitriptyline to 50 mg/day.
- Day 4-7: reduce dosage of amitriptyline to 25 mg/day.
- Day 12: stop administration of amitriptyline.
Start bupropion
- Day 14: start administration of bupropion in a dosage of 150 mg/day.
