• Day 28: start with 12,5 mg at night
• Day 29: 25 mg in divided doses
• Day 30-42: increase daily dose every 1-2 day(s) with 25 mg
• Day 43-55: if necessary, increase dose with 50-100 mg/week

• Increments are dependent on the status of patient
