Switch medication from mirtazapine to fluvoxamine.[1] [2]

  • Day 0: gradually reduce dosage of mirtazapine to a maximum of 30 mg/ day.
  • Day 1: reduce a dosage of 30 mg/day to 15 mg/day.
  • Day 8: stop administration of mirtazapine.

  • Day 8: start fluvoxamine in a normal dosage of 50 mg/day.
  • If necessary, increase dosage of fluvoxamine to 50-100 mg/day.

  • Caution with this switch is necessary, because fluvoxamine slows the metabolism of mirtazapine via CYP1A2, CYP3A4 and CYP2D6.
