Switch medication from clomipramine to duloxetine.[1] [2]
- Day 0: gradually reduce dosage of clomipramine to a maximum of 50 mg/day.
- Day 1-7: reduce dosage of clomipramine to 25 mg/day.
- Day 8: stop administration of clomipramine.
- Day 8-14: a wash-out period of one week is necessary.
- Day 15: start administration of duloxetine in a normal dosage of 60 mg/day.
- Occurrence of serotonin syndrome is theoretically possible, so caution is necessary.
- Duloxetine is an inhibitor of CYP2D6, which metabolizes clomipramine.
