Switch medication from venlafaxine to trazodone.[1] [2]
- Gradually reduce dosage of venlafaxine to a maximum of 75 mg/day, when this dosage is > 75 mg/day.
- When a dosage of 75 mg/day is reached, stop administration.
- Day 1: start administration of trazodone the next day in a dosage of 300 mg/day
- If necessary, increase dosage of trazodone to a maximum of 400 mg/day.
