  • Gradually reduce dosage of fluoxetine to a maximum of 20 mg/ day, when this dosage is > 20 mg/day.
  • When a dosage of 20 mg/day is reached, stop administration.

Caution is necessary.

  • Day 1-35: a wash-out period of five weeks is necessary.
  • Day 36: start tranylcypromine the next day in a normal dosage of 20 mg/day.

  • A switch from fluoxetine to tranylcypromine is not obvious, given the necessary prolonged wash-out period in this case.
  • Occurrence of serotonin syndrome is reported when a wash-out period (of five weeks) is not considered.
