Stop venlafaxine - Gradually reduce dosage of venlafaxine to a maximum of 75 mg/day.
- When a dosage of 75 mg/day is reached, stop administration.
Start paroxetine- No wash-out period is needed.
- Start paroxetine the next day in a normal dosage of 20 mg/day.
