Switch medication from clomipramine to paroxetine[1][2]
Stop clomipramine - Day 1: Decrease with about 25% of the original dose per 3 days.
Start paroxetine- Day 7 and after: At approximately 50-25% of the original TCA dose start paroxetine at 50% of the target dose.
- Day 14: Increase paroxetine to 100% of the target dose

- paroxetine is a strong CYP2D6 inhibitor, which metabolize (desmethyl)clomipramine.
