Switch medication from clomipramine to venlafaxine.[1] [2]
Stop clomipramine - Before day 1: gradually reduce dosage of clomipramine to a maximum of 50 mg/ day, when this dosage is > 50 mg/day.
- Day 1: reduce dosage of clomipramine to 25 mg/day.
- Day 8: stop administration of clomipramine.
Start venlafaxine- Day 8: Start administration of venlafaxine in a normal dosage of 37,5 mg or 75 mg (slow release) /day.
- Day 15: increase dosage of venlafaxine.
