Switch medication from trazodone to venlafaxine.[1] [2]

  • Before day 1: gradually reduce dosage of trazodone to a maximum of 150 mg/day, when this dosage is > 150 mg/day.
  • Day 1: reduce dosage of trazodone to 75-100 mg.
  • Day 8: stop administration of trazodone.

  • Day 8-13: a wash-out period of about one week is necessary.
  • Day 14: Start administration of venlafaxine in a normal dosage of 37,5 mg or 75 mg (slow release) /day.
